As we head into the fall season, it’s time once again to protect ourselves and our community against illness. We’re excited to announce two upcoming flu and COVID-19 immunization clinics that will help you stay healthy this winter.

Time: 9:00 a.m. to 1:00 p.m.
Location: Thomas Hall – Tahoma room

Clinic #2

Organization Code: lnatpd
Date: November 4, 2024
Time: 10 a.m. to 2:00 p.m.
Location: Wheelock – Rotunda

Cost to Participate:
If you are enrolled in the university’s health plan, there will be no out-of-pocket cost to you. Insurance will be billed directly. Self-pay will also be available for faculty and staff who are not enrolled in the university’s health plan. The cost is $32.00 per vaccine up to age 65 and $94.99 if you are 65 or older.

Appointments:
Appointments are required and can be scheduled using the link above. You will need the unique organization code to sign up. Please use the code that corresponds to the date of the clinic you want to attend.

Please bring with you:

  • Your health insurance card
  • Photo ID (driver license or other government issued ID)
  • Vaccine Record card
  • The latest version of your Medicare card, if applicable

Immunization Consent Form:
Please complete the attached immunization consent form and bring it with you to your vaccination appointment.
